ONGOING LESSON FEEDBACK
This form is here to help keep an open line of communication between our MJMS students and parents, and our instructors. Please consider filling it out every few months to communicate with your instructor about what is going well in lessons and what you would like to change or improve. Of course you can (and should!) speak with your teacher in person as well, but we hope you will consider this an opportunity to talk about your (child’s) lesson experience in more depth.
Once you submit this form, it will be shared directly with your teacher and they can discuss it with you at your next lesson.
