MAKEUP LESSON PASS REQUEST

Each MJMS student is issued two makeup lesson passes at the beginning of the studio year. Makeup lesson passes are valid for lessons on any instrument with any teacher on a first-come, first-serve basis, and may be used at the student’s convenience any time during the current studio year. Please complete this form to request a makeup lesson pass!*

*A makeup lesson pass will not be issued if 24 hours’ cancellation notice has not been given. Tuition will not be refunded for lessons missed by students.
